110-301-0331(2) During the course of the complaint assessment, licensor Mulloy obtained enough information from staff and collateral contacts to determine a supervision violation occurred. A child with know behavior conserns who has an IEP and was on a behavior plan created by the program was teasing another child,chased them and then stabbed the child 3 times in the leg with the pencil.
