Observation

During the course of the complaint assessment, Licensor Champney obtained enough information available to determine a child had locked him/herself in the bathroom; out of frustration the child banged his/her head on the bathroom wall. RAS Staff did not have any access (bathroom key) to promptly assist or redirect the child as necessary. Therefore, the child was left unsupervised not knowing what the child was doing at all times.
