Observation

Based on the interviews with staff and parents, during this self reported complaint inspection process, DCYF determined finding to be VALID, due to lack of adequate supervision. A child was injured, as child fell from a chair, bumped head, nose and eye which led to child getting a cut on the nose from that fall. Supervision WAC 110-300-0345 (3) (c), is determined to be Valid.
