WAC 110-300-0475(3): The incident report was not provided to the parent on every occasion when the child sustained an injury. The incident report itself did not meet the required standards, missing a signature section and failing to cover all necessary details of the injury. WAC 110-300-0475(2): The program failed to report to the department when the child sustained an injury to his head, and the parent took the child to urgent care and later to the emergency room.
