Observation

The Provider and two staff members did not see the incident or what caused the two-year-old bleed during child care. Staff reported having her back towards the 2 children and engaged in other activities (cleaning up toys) when the incident occurred. The Provider stated the outdoor surveillance footage did not capture the incident. WAC 110-300-0345(5)(c)(vii).
