Observation

The staff member reports the injured child gave him the ice pack after he was done using it, and he thought he could leave it on the ground to clean up later because he was preoccupied with other children. The ice pack was then found by other children resulting in a child drinking the liquid. The ice pack was potentially hazardous and supervision should of been maintained for all children coming into contact with it due to the potential risk. The ice pack and surrounding children were left without active supervision leading to the resulting incident.
