Observation

Based on the information gathered at the site visit, and interview with the director and staffing with the licensing supervisor, it has been determined that the child care center should have reported the child's disclosure of physcial abuse - marks and bruises on his arm from his mother. The child disclosed to the teacher and the teacher did report to the department lead and the director/owner did see the report but no one called intake to report the disclosure.
